Care Coordination and Education |

As
a member of the Asthma Basics for Children Coalition (ABC)
and WIN For Asthma, NMIC provides community outreach and
education on asthma triggers, as well as treatment and
case management services to families with children that
have excessive hospitalizations and school absences. |

Services: |

Provide case management services including home
environmental assessments, referrals, and asthma
education.

From July 1, 2008 to June 30, 2009 we provided case
management services to 34 families.
-
During the six moths follow-up, 95% of the families
reported reducing the number of triggers.
